Medications should be given at home whenever possible, however, if medication is needed during the school day, the forms and information needed can be found below. Medication forms can also be picked up at the school or at your student’s clinic.
In order for school staff to administer prescription medication, the following must be on file:

In order for school staff to administer a non-prescription medication, the following must be on file:
- Written consent from the parent/guardian (found on the form below).
- Medication must be in the original container.
Non-Prescription Medication Form
The district does offer acetaminophen, antacids, cough drops, and ibuprofen on a limited-term basis for students who may need it throughout the school day. Parent/guardian consent is required prior to a student receiving this medication. The consent form can be found below:

Please keep in mind the following general medication reminders:
- Students may not carry medication at school with the exception of prescription inhalers and epi-pens
- In order for students to carry their inhaler/epi-pen, they must have parent/guardian AND provider written permission.
- Please deliver any medication to the health office. Do not send it with your student.
- A new medication form is needed each time the medication is changed and at the start of each school year
- Expired medication is disposed of unless otherwise directed.
